Before Your Care with TP Care at Homes Ltd Begins

We believe in being open and transparent about how we work. Before we start delivering your care, we follow some important steps to make sure your needs are fully understood and that your care is safe, effective, and personalised.
1. Initial Contact
We arrange a visit with you, either in your own home or in hospital if necessary, to discuss your individual needs and the services we can provide.
2. Assessment
Our Registered Manager (or a member of the team) will complete a detailed assessment with you. This covers your health, personal preferences, daily routines, and goals. If specialist knowledge is needed, our Registered Nurse will review the request to ensure the right skills are in place.
3. Care Plan
Using the assessment, we create a personalised Care Plan that sets out exactly how we will support you. You will be fully involved in this process, and your views, history and preferences will shape the plan.
4. Agreement and Consent
You will be asked to review your Care Plan and sign it to confirm your agreement. This also gives us your consent to begin delivering the care and support as planned.
5. Care Team Allocation
We match you with a small, consistent team of Care Workers who are best suited to your needs. We make sure you know who will be coming, when they will visit, and how long they will stay.
6. Information Sharing
Your Care Worker will use your Care Plan to guide their visits. They will also record important details from each visit, so we can monitor your wellbeing and adjust your care if needed.
7. Review and Ongoing Support
Within six weeks of starting your care, we will carry out a review to ensure your Care Plan still meets your needs. Reviews will then take place at least once every 12 months, or sooner if your situation changes.
