Continuity of Care

Why continuity matters
One of the most common worries families have about home care is meeting a different, unfamiliar carer every visit. At TP Care at Homes, continuity isn't an afterthought — it's built into how we plan every package of care from day one.
A regular, familiar team
Wherever possible, each client is supported by a small, consistent team of carers who get to know their routines, preferences and personality — not a rotating cast of strangers. Building that familiarity is, in our experience, what makes care feel less like a service and more like support from someone who genuinely knows you.
When plans change
Carers do need holidays, and illness happens. When it does, we plan cover in advance wherever possible and make sure any covering carer is properly briefed on the client's routine, needs and preferences before they visit, so care standards stay consistent even when the usual carer isn't available.
Clear handover, every time
Every visit is logged, and any covering or new carer is briefed using up-to-date notes — so nothing important gets missed, and clients and families never have to repeat the same explanations to someone new.
Staying in touch with family
We understand that continuity matters to families too, not just clients. Where agreed, we keep family members updated on how care is going, and we're always reachable by phone or WhatsApp for anything that comes up between visits.
